Jacob Davis DNP, APRN, FNP-C, CLS
Jacob Davis was one of the first nurse practitioners in the nation to be awarded diplomate status with the Accreditation Council for Clinical Lipidology (ACCL). For the past six years, he has worked under Dr. Grant Craig at Advance Medical Group, where he has developed a passion for managing chronic illnesses, including diabetes, cardiovascular disease, and lipid disorders, as part of comprehensive primary care and renal care services. Outside of family time, Davis enjoys reading, attending concerts, traveling, and horseback riding.
Leslie Jackson MSN,APRN,FNP-C
Leslie Jackson is a Family Nurse Practitioner with more than 20 years in healthcare.
Leslie is a life long Grayson County resident, so a lot of her patients are neighbors, friends, or people she’s known for years. That shapes how she practices. Leslie listens first, explains things in plain language, and builds a plan with you instead of handing one down. No lecturing, no judgment, and no rushing you out the door.
She has a particular interest in weight management, hormone replacement therapy, and mental wellness. Her care is evidence-based but practical: what will actually work in your life, not just what looks good on paper.
Outside the clinic, Leslie is with her family, traveling when she can, playing pickleball, and always enjoys a good laugh.
Dr. Mandeep Singh, MD
Medical Director – Texoma Dialysis Center
Dr. Mandeep Singh, a board-certified nephrologist and Medical Director at Texoma Dialysis Center, is committed to advancing patient care through the Advance Medical Group. He earned his medical degree from Government Medical College in Amritsar, India, completed his Internal Medicine Residency at SUNY Stony Brook, New York, and pursued his Nephrology Fellowship training at Methodist Medical Center in Dallas, Texas.
Specializing in the diagnosis and treatment of kidney disease, hypertension, and dialysis care, Dr. Singh focuses on delivering compassionate, patient-centered renal care. His dedication lies in helping patients achieve the best possible kidney health outcomes and emphasizing the importance of primary care in managing renal health.
